You shouldn't have to read a regulation to figure out which form claims your money. Here's the short list a PCS actually runs on — what each one does, what you need to fill it out, and a link straight to the official version.

DD 1351-2

Travel Voucher or Subvoucher

The form you file after you arrive to actually get paid for your PCS — mileage (MALT), per diem for travel days, lodging en route, and the lump sums like DLA. If money from your move is going to reach your bank account, it goes through this voucher.

What you'll need

  • Your PCS orders and every amendment
  • Lodging and travel receipts (anything $75+, plus all lodging)
  • Weight tickets, if you did a PPM/DITY
  • Your dependents' travel dates, if they moved with you

Who signs

You sign it; your servicing finance/disbursing office reviews and pays it.

Where it goes

Your finance office / DFAS — often submitted through DPS or your travel system.

DD 1561

Statement to Substantiate Payment of Family Separation Allowance (FSA)

Starts Family Separation Allowance when the service keeps you away from your dependents for more than 30 continuous days — a common PCS situation when you report ahead and your family follows later. It's a flat monthly allowance (long set at $250/month).

What you'll need

  • Proof of dependents (already in DEERS)
  • The dates your family is separated from you
  • Your orders showing the move/assignment

Who signs

You certify it; your command/admin verifies and forwards it.

Where it goes

Your admin/PSD, then finance to start the entitlement.

DD 2278

Application for Personally Procured Move (PPM) and Counseling Checklist

Authorizes a personally procured move (PPM, formerly DITY) and locks in the government's cost estimate — the number your incentive payment is based on. No DD 2278, no PPM reimbursement.

What you'll need

  • Your orders
  • An estimated shipment weight
  • Origin and destination addresses
  • A counseling appointment booked through DPS (move.mil)

Who signs

You and your origin Personal Property / Transportation Office.

Where it goes

Your Personal Property Office — generated and submitted through DPS.

DD 1299

Application for Shipment and/or Storage of Personal Property

The application that authorizes the government to ship and/or store your household goods at government expense — the HHG/DPS path, where movers do the lifting instead of you.

What you'll need

  • Your orders
  • Pickup and delivery addresses (or storage details)
  • Your DPS self-counseling completed

Who signs

You sign after counseling; your Personal Property Office processes it.

Where it goes

Your Personal Property Office — generated through DPS (move.mil).

DD 1797

Personal Property Counseling Checklist

The record that confirms you were briefed on your move entitlements and responsibilities. DPS generates it during self-counseling — you print, sign, and date it so your shipment can be booked.

What you'll need

  • Your DPS self-counseling completed
  • A printer (you sign and date a physical copy)

Who signs

You sign and date it after counseling.

Where it goes

Submitted with your shipment application in DPS (move.mil).

DD 2058

State of Legal Residence Certificate

Sets or changes the state your military pay is taxed in — your state of legal residence (domicile). Filing it can stop the wrong state from withholding income tax from your check. Worth a look any time you PCS across state lines.

What you'll need

  • The state you're claiming as legal residence
  • The facts that back it up (where you vote, register cars, hold a license)

Who signs

You; it's filed with your finance/pay office.

Where it goes

Your servicing finance/pay office (often via your admin or myPay).
